For over twenty-eight years, Project Transition (PT) has provided help and hope to adults with psychiatric problems and co-occurring disorders that have been recurrent and persistent. Many of the people we work with have felt failed by previous treatment efforts, including psychiatric hospitalizations, medications, and drug & alcohol rehabilitation programs. Project Transition’s unique apartment-based therapeutic communities are supportive and caring. We promote sustainable recovery which includes the treatment and self-management of psychiatric difficulties (with or without coexisting substance abuse), the restoration of functioning, relationships, and self-direction and reintegration into the community.
Project Transition cultivates trust and hope through relationships, treatment, skill development, wellness training, and supervised independent living. Our therapeutic communities are based within attractive apartment complexes in southeastern Pennsylvania (Bucks, Montgomery, Berks and Philadelphia Counties). Roommates share a fully furnished apartment; they have access to a fitness facilities, shopping, and leisure attractions. Program activities and services, which are provided at the apartment complex, are augmented by our 24/7 crisis response team (counselors, program directors, and psychiatrists). Family activities include a welcoming orientation, ongoing progress meetings, multi-family support meetings, and educational seminars.
Our staff team consists of experienced professionals: Psychiatrists, Psychologists, Psychiatric Rehabilitation Counselors, Program Managers, Addiction Counselors, and Wellness Specialist. Most of our clinicians have over twenty years of experience.
